• Please make checks payable to: GUTHRIE-JENSEN CONSULTANTS INC. Guthrie-Jensen is a registered enterprise with TIN 00-121-912-000
• All cancellations/rescheduling made less than ten (10) days prior to the first day of the seminar are subject to a Php3,500 fee per participant for 2-day programs, Php7,500 for the 3-day programs, and Php12,500 for the 5-day programs.
• Non-appearance to the seminar is subject to a Php3,500 fee per participant for 2-day programs, Php7,500 for the 3-day programs, and Php12,500 for the 5-day programs.
• By submitting this form, the sender agrees to these terms and conditions. Guthrie-Jensen will also send regular updates via email and traditional mails.
